One team around a personnot one queue per problem

Youth and out-of-home care, disability and NDIS, aged and community care, family preservation. Four program areas, one plan, one record — so nobody has to tell their story four times to get help once.

Who this is for

The people whose lives do not fit one funding category

A young person leaving care who also needs an NDIS plan. A grandmother raising a grandchild who needs home care herself. The join is where support usually falls apart, and it is the part we organised ourselves around.

People supported across the four program areas last year
240
Staff, and 60 authorised carer households

Our approach

Tell it once

By the time someone reaches us they have usually explained themselves to a dozen agencies. We try to be the last one that asks. That means a plan the person helped write, a named worker, a colleague who already knows the situation for when that worker is on leave, and a relationship that outlasts the funding period it started in.

Relationship first

Support only lands once someone believes the person offering it will still be there next month. We roster for that, not for throughput.

Evidence produced once

Four regulators, twenty-seven funding agreements. The proof comes out of the work, not out of asking people to repeat it.

Culture is the work, not a section of it

Aboriginal families decide what connection to kin and Country looks like for their children. Interpreters are booked by default, so nobody has to ask for one in a language they are struggling in.

The difference it makes

Measured where it counts, not on the day someone left

We check at six and twelve months, because that is when you find out whether it held.

94%

Get in touch

When in doubt, ring us

A few minutes on the phone settles whether we are the right service. If we are not, you will leave the call with the name of someone who is.
